Help! Beagle has two bald itchy patches on back.

Does the dog squeeze thru any tight spot in fence that could possibly get it wood splinters or other foreign object embedded to cause infection ?

I was given a puppy that had mange. The vet treated him with Advantage or Revolution. Anyway it was of the oral flea medications. Your dog could also have ringworm. I once had a litter of barn kittens with ring worm and the vet had me use lime sulfur on them. I think you get lime sulfur in the garden department. I had to mix up a solution and dip the kittens in it. It worked.
