Help! Eye discharge and weak chicken

My hen has crusty eyes. No discharge from the nostrils. She's thin and weak. (I'm hoping it's just from no eating and drinking from eyes being crusted shut. I have cleaned them and got them open. Here are pics. Any ideas of what's going on?
