Is this something that needs to come out or push in?

You don't want to push it back in because it's a big pus ball caused from infection.

I'm not sure how common this actually is, and I myself didn't learn about it until a friend told me a couple weeks ago and I read about it.

I'd leave it and wait to see if your hen removes it on her own. If not you could very gently try to remove it yourself. Because it's a pus ball she shouldn't feel it since there's no nerves in the lash egg itself but still you don't want to tear her cloacal wall either

I am not familiar at all with this.

Would starting her on antibiotics help the infection to go away?

Not really. You can but from what I've read by the time you're seeing these pus eggs there isn't much to be done.
