He has broken all these rules. My heart breaks for you. Thirteen years is a long time, and a lot to throw away, but don't cross it off your list unless things improve. There is more than just infidelity and hitting that constitutes abuse.
Counseling will help if he is willing to participate - it takes a commitment to make it work. I recommend a real counselor as opposed to clergy. Someone trained in family counseling (at least a Masters or PhD) will have more effective tools than praying and preaching morals (though these things have their important place in many peoples lives). In my OPINION, his actions toward you suggests a real lack of respect...and a lack of respect is a symptom of trouble brewing.
