Other than XF2.2, what do all of the forums have in common? Is it possible that all of the forums are also using Cloudflare?

I don't think they are all using Cloudflare.

I think the two are related.

"Unassociated" attachments are automatically deleted after a while, so it'd be good to know if the "not found" errors ever occured first, or the permission errors always occured first, and all of the "not found" errors are simply due to the unassociated attachments being deleted.
