If she can't successfully pass the egg, then breaking/extracting the egg would be necessary. As @aart mentions - it depends how this is done. Yes, there's a possibility that egg shell can cause cuts which can get infected. There's a possibility that egg matter can travel back up the oviduct, drop into the abdomen causing infection or later down the road Peritiontis, Salpingitis or some other infection of the oviduct/reproductive tract.
There are quite a few possibilities to this scenario, but the bottom line is the egg needs to come out. IF the OP feels like there is risk of infection upon extraction, then it may be appropriate to consider antibiotic treatment. A vet will be able to provide a script the correct antibiotic.
