Levamisole wormer info needed

If we can come up with a dosage, I can just weigh up the amount on my gram scale? Cool! Then do I just mix the amount in some baby food and down the throat it goes? And is this a 1 dose thing or is it repeated for a few days?

I am not sure if I'm going to treat the entire flock or not. I have one hen that has had Capillary worms in the past from a poop sample at the vet and is showing signs again. Currently I am using Safeguard on her, we are on day 4. Poop looks fabulous, but it goes bad again quick after the worming session is over. This is why I wanted to try another type of wormer. And need to know how to use this stuff. I have heard it can be quite toxic if not used properly.
 
If you can weigh 1.1 grams that will give you 1000 mg of levamisole. You could try mixing the 1.1 grams with 20 ml of water and that will give you 20 ml @ 50 mg/ml.

Anyway, if you decide to go with the max that the PDF you found:
5 pound bird x 16 / 50 = 1.6 ml per five pounds
 
So then would I give her this 20ml@50mg/ml in one dose?
