Would it be ok to lay the heating pad down with cloth on top of it instead of the cave?
Yes.
What would be better though is to put it up against the side of your brooder. So they can lean against it if they want.
I find it really hard that they would need 90 degrees right now. I had quail chicks completely off heat in one week.

Good idea! Well 2 of the chicks are still tiny. Only the other blue actually looks the age. The larger blue usually sleeps half out of the cave and the smaller 2 sleep in it

If she had a bad case of coccidiosis for a while it could have messed her intestines up. There is no way to know this though and I'm not sure there is a way to fix that.
The best thing is to provide good fresh feed and perhaps vitamins.

She actually doesn't appear to have grown much at all. My other two have. Only difference is the feather texture is different than before. I clean and refill their food, water and bedding every morning. Could I use Save a Chick?
 
She's a 5 star hatchery, but maybe she just goes in and deletes bad reviews, who knows. Should I put the body in the refrigerator to help preserve it until the morning?
Yes, definitely refrigerate it, but seal it in a ziplock first, maybe two, just in case.

EDIT - unless, of course, if this is the chick you thought was dead but isn't. In that case, I definitely would NOT recommend a ziplock baggie! :duc
 
She's a bit better, but still wobbling around. I don't think she'll live through the day but she's already come back once :fl
Having Nutri-Drench on hand is good for chicks like this because it is absorbed into their system fast.
Did you ever get some of that, sorry I can't remember.
