Repeated poopy butt

I have a RIR hen that keeps getting a poop clump on her butt. I cleaned off a big glob of poop on her butt. I had to cut some of her feathers off it was stuck so bad. It is back a week later. Someone locally suggested it may be a yeast infection and to try monistat, but I thought I would ask here first. The photo is the first time, but it is the same.

It's possible that it could be vent gleet. Here are some links (with photos) that have treatment info. I would definately get her cleaned up really well so you can inspect the vent and surrounding tissue to make sure she doesn't have fly strike starting as well.

Here are some short videos of Terri Golson from the HenCam, she has really simply techniques that may help you. I think you would have white discharge is it is vent gleet. Have you wormed her? Sometimes it could be parasites or even nutrition. Some hens just get messy bottoms too. But I would look clean it up and look closely at the skin and vent, just to make sure.

Thanks. Hopefully I can get home in time today. It makes me wonder if she is the pile on the roost and just sits in it. We shall see.

Just another thought thrown in here. It's been a while since I viewed Terri's videos. You could just let her sit in the spa treatment http://hencam.com/faq/the-spa-treatment/ not give her a whole bath.
Does she have access to a dry patch of dirt to dust bathe? Does she roost or does she sleep on the floor?

They have dirt and they roost over a poop board, but one spot where the ladder comes up under the one board someone roost there and poops a pile. I clean the pdz/sand once a week
