Skinny, foul poop, sick chicken

Hello everyone...could I get some feedback / thoughts? I am sorry, I don't really know how to tag people yet!

Today, I brought the hen in for a soaking of her feet. She pooped four times in like a half hour. I took pictures of them. The first picture was watery with solids mixed in. I see lots of brown grass strands. She was out foraging yesterday for a little bit. The second poop a few minutes later was all brown liquid. I had my magnifying glasses on and noticed WORMS MOVING!!!! I could not see them at all without the glasses on. Her 3rd and 4th poops were in the tub and I think it basically looked similar to the first poop. So, I notice that the horrible foul smell of the poop is not there any longer. But I wormed her with Ivermectin pour on on 12/17, again on 12/31. Now I see moving worms. What should I do? What do you think about the poop pics? We are still doing fish mox and fish zole. Do you regularly use Ivermectin to worm your flock as a standard wormer? It may be the worms have become resistant to it, and you need to alternate worming medicines.

No, Prior to my hen getting sick, I have never wormed them since I got them two summers ago. I would give the DE mixed with their food sometimes because I had heard this was supposed to help worm them. But now I am reading that it doesn't really help. It has been two weeks since the last ivermectin treatment. Wonder if I should treat again or use another kind. Or if that is normal after worming.
 
Have you ever had a fecal float test done on the poop? That's what I recommend so you know what you're dealing with. Any vet can do it. Identifying the worms you see with the magnifying glasses should let you know what wormer you might need to try next.
 
Well, It has been a month since my original post. Her weight last night was 4pounds. SO she is slowly gaining weight and she is roosting at night with the rest of the girls again. However, she is still not having solid poops. I stopped the antibiotics 2 days ago. And started putting probiotics in the water. What do you think?
