Weird hole in egg shell?? (Pics)

HI. Can anyone tell me what causes this abnormal hole in the egg shell. The inner membrane is not broken and the yolk/white are ok. On the outer edge of the hole there's a raised calcification. Just curious, I get one egg like this each month. THANK YOU!

The hen just didn't finish. I've found those and I've found eggs with little bumps on them that, if you pick it off, will expose the yolk. Again, it is just one of those things you'll find on BYC eggs that get weeded out on commercial eggs.
 
i've picked away the shell at the hole's edge and there doesn't appear to anything wrong with the rest of the egg. I also get those bumpy ones from time to time; just didn't know if there was a cause for this?
 
We feed 16% layer pellet and 20% broiler pellet mixed at 2/1 ratio to bring up the protien for winter. We also offer ground meat and bone free choice at 60% protien/ 40% calcium
 
I got several eggs like that they just looked weird and I ate them no problem!! and some with the calcification all over the egg. didn't seem to hurt anything and the hens are ok they got less as they got older.
