Why are the ears/cheeks so swollen? the eyes are also not wide open

The more detail you can provide about the symptoms you bird is showing, what changes have taken place, and when they started, the easier it will be for anyone to try to identify what the problem is.
 
Can you take a closeup photo of this beautiful hen's swollen features and post it. The video is great for seeing her behavior, which indicates she may not be feeling well, but we need to see her face to see the swelling.

It sounds like a common respiratory infection, perhaps Mycoplasma gallisepticum. It's usually treated with the antibiotic Tylosin.

I still believe she's feeling miserable from a respiratory infection. Try to find a vet that will help you get some Tylosin for her. It comes in a powder you add to the drinking water.

I am giving injectable combination of Tylosin and Gentacin. Is that fine? The tylosin that I have whenever i give the chicken a shot, the next day, it is dead!
 
Wow! I wish we here had access to such a selection of antibiotics.

No, she should do well on what you're giving her. I expect with your expert care, she will be back to chicken business in no time.
