Maybe wry neck? I have done all the supplements and she’s still eating/drinking watch the video and fast forward to 3:24 to see what I am dealing with, I really need some advice as to what to do with this poor girl!!

What have you been supplementing with? How much, how often, and how long?

How old is your chicken? Was she vaccinated for Mareks disease? Could she have a head injury? It looks like wry neck or torticolis, which is a neurological symptom. Head injury, vitamin E or thiamine (B1) deficiency, or certain disease can be possible causes. Those include Mareks and some rarer diseases such as avian influenza and Newcastles. Where are you located?

Most people give 400 IU of vitamin E and a source of thiamine, such as 1/4-1/2 tablet of vitamin B complex daily for a couple of weeks to see if symptoms improve. The chicken may need to be hand fed if she stops eating and drinking. Hopefully, her symptoms will improve. Watch to see if she needs protection from others, since they sometimes get attacked when acting unusually. Here is a video of feeding a chicken with wry neck:

Don't worry about her. The vitamin E and B-complex will fix her up. You can also add turmeric if she's slow to respond.

We did the vit E and selenium for a few weeks with no improvement. Upstate NY and yes she’s vaccinated

My one experience with what turned out to be a vitamin deficiency caused wry necked chick didn't turn out well. I had her necropsied, and she had severe brain lesions, and would never recover. Euthanasia was a kindness in her case.
She was necropsied because I was worried about a possibly contagious cause, fortunately not the problem.

Recently, my very, extremely old hen Lady Di developed wry neck from the tumor she had on her head for half her life. I started treating her with 400iu E and as time passed, she needed more, so I bought the E that's bigger and stronger. The E worked fine to get the wry neck under control so she could go back to eating on her own.

This would have worked out fine if it weren't for the tumor pressing on her brain. She finally reached the point where nothing helped and I had to euthanize her.

Keep trying more E and do it twice a day. E works. But as @Folly's place has pointed out, underlying factors that are working in opposition to treatment need to be reckoned with. Keep trying until it's no longer working.
