chicken has dislocated leg and is breathing heavily!!!

hello everyone, need desperate help and fast please!
one of our road islands had dislocated its leg yesterday so we took it in over night fed it some food and water. well now its breathing really heavily, closing its eyes now and then and drops its head very slightly.
PLEASE PLEASE HELP URGENTLY
