My bigger guess would be contamination from poo and stuff getting on the egg during the incubation process.Is lack of humidity during the last few days often a problem with a broody hen doing the hatching?
Or it's possible mama wasn't patient enough and tried to help them out of the shell. But I would expect to see a little blood then, per my own previous mistakes.
Like you say, hard to know for sure.
