HELP! Four-Week, Old Chick, Watery Poop...

New chicken owner needs help please. I have seven 4 week old chicks and just noticed one that is very lethargic, and can barely move. Doesn't want to stand up. I cleaned her butt and she immediately had watery poop. She is not peeping but has eyes open. After I cleaned her up and after her poop she did peep a little and drank some water. What do I need to do? I separated her from the others with her own water and food. Any help would be appreciated. Thank you!

Forgot to say that they are on medicated feed and have been vaccinated for Marek's Disease. Thanks!

She looks bad now. Can barely move. Had another watery poop. She won't eat or drink. The other chicks are fine so I'm wondering if it's something other than the heat. If you know something I could try, please let me know.
