Lethargic waddling Chicken

I have chicken that I noticed would leave the perch today. I took her out and noticed she was waddling. She was able to run, but then she would fall to her side and catch herself with her wing, as if she lost balance. Whenever she was at a stand still she began to bob her head and go to sleep. I saw her poop once and it was runny. I have her separated her with a dish of food and plenty of water. Is this nutritional, disease, or something else?

Can you post some photos of your hen and her poop?
What do you feed, including treats?
How old is she?

Hard to know what's going on with her. Monitor her and check to make sure her crop is emptying overnight, look her over for lice/mites, feel her abdomen below her vent between her legs for bloat or fluid. Do look for any indication of injuries, wounds, swelling of the feet/joints, bottom of feet too and for raised scales on the legs. Consider getting a fecal float to see if worms are part of the problem.

See that she's able to get to food and water and watch to see that she's drinking well.
She may benefit from a direct dose of poultry vitamins for a few days.
