and I need to know how much Vetrx to add to add to how much water and how many drops to administer? Thanks! Unfortunately I have to get it through the mail so it will be a while before I can help the poor guy :(

And as a side note I have seen that you're supposed to put some of the solution on the cleft of the chicken's mouth but I've seen people on here talking about dropping it directly into the chicken's eye which would be easier and work better (I would think).

How do you know it has eye worms which from my readings is rare and occurs in tropical regions. Could you share a picture?

It is labeled for "eye worm" (with the quotes, but I have no idea why "eye worm" needed quotes but scaly legs did not), but I have to admit that I am skeptical by looking at the ingredients.
I have seen other people on here talk about having success with it here

Here is an article on eyeworm that says you can use VetRX, but must rinse it out of the eye after using: http://www.chickenwhisperermagazine.com/articles/do-your-fowl-have-mansons-eyeworm
I have never had to treat for eyeworm, so have no personal experience.
If you also have a bad smell with the swollen face it could be infectious coryza. http://www.thepoultrysite.com/diseaseinfo/82/infectious-coryza/
I've no personal experience with that either, just know from reading.
this is the first time anything like this is happening to me and I'm just trying to gather information at this point and hope the medicine works
