Older hens killed pullets

we had 4 pullets about 8 weeks. They’ve all been integrated for a few weeks. Things were going well. Not much bullying just the odd peck here or there. Yesterday we found two pullets scalped. One dead and one almost dead. I can’t say it was 100% the older hens but that’s what I’m guessing. When I checked this morning she was gone 😭 they were the sweetest chicks! I’m not sure what to do with two other pullets. I pulled them for the night in fear of them getting attacked. Do I get more chicks to make a larger second flock? I thought two wasn’t enough. How long to keep them separated?
 
We have four pvc pipes for eating inside the coop. And one hanging 5 gallon water with four drinking cups. Large attached run
 
Photos of the set up overall would help. Even after integration you still need clutter as well as feeders spaced apart to prevent fighting over resources.

Personally I would assume predators or accident were the cause of the original injuries, unless you actually see the hens initiating the attack. It's normal if the hens start pecking at injuries after they happen, but does not mean they were the initial cause.
