Yes, I'm in Canada, but I haven't dealt with lice, yet. Carbaryl "poultry dust" would probably be the best choice here.

Dusting Powder 5%
Indications:
A formulation containing 5% carbaryl, Dusting Powder is indicated to kill ticks, lice, fleas, and hornflies on cattle, sheep, hogs, and horses; Northern fowl mite, chicken mites, lice, and fleas on chickens, turkeys, ducks, geese, and domestic game birds.
Chickens, turkeys, ducks, geese, and domestic game birds:
- For litter treatment, apply 122 g/m². Use a scoop and scatter, or apply by dusting.
- Avoid contaminating nests, eggs, feeding, and water troughs.
- For application to birds as a supplement to litter treatment, use 4.5 g/bird. Apply by shaker can or hand duster. Direct the dust to vent and fluff areas.
- Repeat in 4 weeks if necessary
I've found this dusting powder in the co op feedstores, TSC farm stores, Peavey marts...even independant farm stores. I attached a link to the DVL website so you can see what they carry. Most of their products, such as vitamins and oral antibiotics seem to be covered in most feed stores I've been in. So far it's been very effective for mites for me. It's decently priced at $18-$24 depending on where you can get it.
Permethrin concentrate is available for livestock, and poultry is on label as well.

Looking at the link, I see it mentions a pesticide licence is required...so I have a sinking feeling that it may not be available in all provinces unless you have a licence. Manitoba & Saskatchewan do not need a licence. Beyond that I'm unsure.
In regards to the eyes, I just picked up a medicated eye drop solution from my Walmart pharmacy. It's at home but I'm almost positive it is polysporin eye drops. It's a pink box. This would be off label...but you're treating a rooster, I can't see it would be an issue since he isn't laying eggs and you're probably not going to be eating him. I haven't tried it yet...but I bought it for a hen I have in case it's needed.
I also use something else for eye infections, but it's off label and unless you have cattle...you're vet probably isn't going to sell it to you. Our old vet suggested to use regular mastitus ointment for eye infections...works incredibly well.
