I'll go out on a limb and say that I think that's what may cause a lot of loose air cells too. Take one of your eggs and throw it all around and shake it like crazy. I bet you can't detach the air cell.
 
I had a loose air cell out of one of my own eggs I had a pullet sitting on that I candled when I took her eggs for the incubator. Pretty sure it can happen a bunch of different ways though.
