I agree, with @azygous at this point I would try the Amoxicillin and see if that helps. She has given good advice. Amoxicillin dosage is 57mg per pound of weight given orally twice a day.
I would also consider de-worming her too- you can use Safeguard Liquid Goat Wormer this can be found at TSC. Dose is 0.23ml per pound of weight given orally once a day for 5 days.

Check her crop every morning before she's had anything to eat/drink to make sure it's emptying.

The easiest and most accurate method of getting any form of meds down a chicken is to syringe it directly into the crop via the esophagus. There are two holes in the throat of a chicken. The one in the center behind the tongue goes to the airway. The other one, on the chicken's right side of the throat leads directly into the crop. By inserting the oral syringe into this second hole, you can easily get meds, water, and even liquid food into a chicken without risking choking the chicken if it gets into the airway.

Chicken has been given amoxicillin for 2 days now. I have mixed it with a tiny bit of nutirdrench and Greek yogurt. Yogurt because every time I’ve used amoxicillin I’ve ended up with a yeast infection and I’m already dealing with a 7 week old silky with sour crop right now. I really don’t want to have this one get it from taking a antibiotic. I do know taking a antibiotic and ingesting yogurt does not affect the antibiotic because I do it every time I take an antibiotic. Yes I’m human but still. :)

So anyhow, how long would I give this antibiotic?

I did see that it looks slightly more interested in food but still not a lot. When I do give it the meds I notice there is thin clear bubble in mouth, prior to giving meds. I do NOT notice this when giving the miconazole to the young 7 week old silkie.

Should I worm? I have not wormed because the poop sample I gave to vet came back negative. I have never wormed the year old chickens. Thanks for the help.
 
If a chicken is very sick, showing severe symptoms, I treat for a full ten days when giving amoxicillin. Five to seven days with moderate symptoms.

I haven't seen any yeast infections crop up (pun!) with amoxicillin, but I'm all for the yogurt as a precaution.
 
Update: The vet called and decided to put her on a broad spectrum sulfa drug. Started that last night. This morning when I went to get her to give her the meds (before it was light out, easier to catch) I noticed her comb and face is a bit redder so that might be good news. The vets weight four days ago was 4.6, last night it was 3.6 pounds. If she looses anymore weight or not improving in a couple days I’ve decided to euthanize.
 
Guess who is eating, preening, frolicking, roosting, and improving? She almost got euthanized. Got a epsom salt bath yesterday. Has been getting morning and night nutri drench, a bit of vitamins and probiotics, a bit of baby food not much, and trimethoprim sulfa 1.7 cc. Started eating bread (yes I know it’s not great) and dried worms, and tuna. Those are things I could toss to it as I can keep shoveling stuff down it’s mouth. Didn’t care much for eggs. I am weighing it tonight to see if there is a weight gain but I believe it actually might make it somehow. Hopefully it will be back to eating the chicken food like the others soon. I will give one more update in about a week or so. I just wanted people to know it is possible to help a very sick hen. I was beginning to loose hope. Although not out of the woods it’s close I think. Thanks so much for those that have helped me :)

Successful is still alive and went into molt after being sick. Comb still dull and shrunk. Egg laying started but up but the eggs were warped at top, now the eggs have started to become normal shape though. Wonder if when they molt the combs become less red. Anyhow so far so good. Had I not harassed the vet to prescribe a antibacterial it would not have made it. It got down the 3.5 pounds.
