Do you have a "cease and desist" letter? If not, I would spend a few $$ and have an attorney help you write one or you can find sample ones online. Just search for "cease and desist letter samples"
Email a copy of the letter to the site host as well as the owner of the site.
Always include a deadline for removing the material.
If the email doesn't work, send a registered letter. Believe me, even if the owner of the site doesn't care, the host is usually great about taking it down.
