Chicken who needs home in Western Washington State

I have a chicken who is about 2 years old, she is crossed-beaked. She is very used to human interaction and is very easy to care for. For food all she needs is a soft food mixture such as Yogurt, Whipped cream, Hummus, Apple Sauce, and regular chicken food that is mixed with water or apple juice. She has no problem drinking water. The only problem she has is she cannot fully clean herself so sometimes she needs a little help. We have to rehome her is because we are moving to a place that we cannot bring her.
