Looks like it's just skin tissue regenerating. I would leave it alone, it will heal in time although will leave a scar (like my eyebrow ring I got 20 some odd years ago).

Thank you. It seems like what is filling in is hard and larger than the original puncture. It seems to be pushing the skin apart rather than bring it together. I can feel the hard extension of it under her skin. I'm just worried in ed it won't heal and will eventually get infected.

It may be pushing out some, it's hard to know - if it's scabbing from the inside out, then I would think eventually that's going to come to the surface.
From what I could see in your very first photo, the wound was deep - almost like it may have entered the body cavity, so it has some a long way.

The dark bit looks like it doesn't belong. If you pick at it, does it bleed?

No it doesn't bleed. It is not connected to the skin. It gets moister, yellowish and stringy when the dry black part is clipped away.

Is it yellow under the scab? Can you post a picture?

Yes, but it doesn't smell. I just posted before and after pictures from today.
