Crochet "Chat"

I need some help! I made this blanket as a gift for a friend. (The pattern is from ravelry.)

So when I went to wrap it up, I noticed a spot that came undone.

So the help I need is two fold. First, how can I fix this. I suppose I could redo the square and sew a new one in. I was hoping someone would know a better way.
Second, and most importantly, how do I keep this from happening again. I am a fanatic about weaving in ends because I feel like you want them to stay in even if you put something in the washing machine. So what the heck can I do to make sure this doesn't happen again?

Not sure how to fix it other than redo the square. I've never heard or reverse crochet! Haha. When I weave in ends, I always try to go one way, then backwards in the same space(trying to see it into the yarn going the other way) and hen back be other way one more time.
 
You could secure the loops by darning. Oversew where it can unloop from and then pick up the loops with a couple of circles of yarn.

That's where you've started from, yes? How long do you leave on your first slipstitch? Do you crochet over it, or darn it in afterwards? If the latter, try leaving a long bit and crocheting over it.
 
First it is a beautiful afghan.

It looks like you used the "magic" loop method to start???? instead of a crocheted chain???? I love the magic loop method to start but you do need to take extra steps to tie and secure it. I usually secure w/ a dab of fabric glue, then weave then another dab If you started all of them w/ the magic loop method you need to make sure your other centers are secure.

The only way I know to fix it w/o replacing it would be to darn as suggested, you may be able to get a good enough looking center if it hasn't unraveled much. You will have to make sure every loose loop is caught and secured w/ the darning thread.
 
I appreciate everyone's responses. I did use that darn magic ring. This is the first project I ever used it on. it does seem so slippery so I stitched over what I thought was a long enough tail. oh well, I guess this is a lesson learned. I would still rather it do this to me than do it after it is gifted.
 
I agree with the others about just inserting the end into a yarn needle and sewing back through the loops then pulling it all tight.

The way I've been securing things extra tight is by putting the end in a needle and sewing up a tight little knot so you can't see it and then either weaving the end in or crocheting over the end.

Yeah those magic rings can be messy in some projects. Square knots are needed but some thread yarn is slippery enough that even a square knot can come loose.
