What kind of disease are you talking about? my flock all live together Ducks goose and chickens and I have never had a problem. I clean my coops everyday to keep them as clean as possible. And they all eat the same feed. They have access to a little over 1/2 acre with fencing surrounding them. They all get along great. Most important if your considering having a drake is to have plenty of female ducks for him. That way he won't be interested in your chicken hens.

I was about to write the same things! Plenty of people have mixed flocks of Chickens, Ducks, Geese, Muscovies, some even have mixed in some Turkeys. No issues with diseases - those will all be shared by everybody. As @Miss Lydia said: Careful with the drakes, comes spring they like to use everybody and everything as pool-noodle…
 
We're giving serious consideration to raising ducks. We will probably need a distant site from our chicken run and coop though so there wouldn't be a problem with any possible disease affecting one another, right?
Ducks are very hardy and don't suffer or spread much illness at all. I've had mixed flocks but I would see the appeal in the ducks having their own run as they are very messy, and no body of water is safe from their muddy noses. So for the chickens sake needs to be fresh drinking water, either in their own run or behind a little fence the ducks can't get over and mess with. I had a foot high fence for mine, easy enough for chooks to jump, hugh enough ducks thought it was impossible
